A Case of Brown Tumors Caused by Primary Hyperparathyroidism Mimicking Bone Metastases

Abstract
Primary hyperparathyroidism is a rare endocrine disease. The introduction of routine calcium measurement brought early detection and treatment of primary hyperparathyroidism, which led to a marked decrease in classical bone manifestations of the disease. We experienced the case of a 30-year-old women presenting as right shoulder pain and multiple osteolytic bone lesions suspicious of bone metastases. Extensive workups including bone biopsy were done. The result of biopsy at left proximal humerus mass was consistent with brown tumor. The intact parathyroid hormone level was elevated and a parathyroid mass was detected by 99mTc-sestamibi parathyroid scan and neck ultrasonography. The patient was diagnosed as primary hyperparathyroidism due to a parathyroid adenoma. After parathyroid adenomectomy, serum calcium and intact parathyroid hormone level returned to normal ranges. This report highlight the strong need to consider primary hyperparathyroidism in the initial diagnosis of multiple osteolytic bony lesions with hypercalcemia.
